This is the second time this has happened to me. The first time was a while ago, and to be honest I'd forgotten about it until now. I was playing Breakneck Triad, and I fell into the lava. As usual, the screen dimmed as the death music played. I waited a second as the regular music started up again. I could hear the timer ticking down for the round to start. I thought, "wow, it's taking it's sweet time now isn't it?". Then I clicked the mouse and realized that I was shooting, that I was alive and well. So I stumbled around a bit in the "dark" and eventually heard the lava sizzle and death music again, and suddenly I could see again! I'd forgotten about this, but it happened again tonight. I got killed by an enemy, and my screen didn't "un-dim" itself. I pressed ESC and VOILA! I could see again. Then I unpaused and OH NO, I was blind again. So that was a little bit irritating.
The short version: I died. Screen stayed black. I can see only when the game is paused. Died again (when blind). Can see again.
This has happened to me in BOTH singleplayer AND multiplayer. I don't have a video this time because... well what would I show?

I was only going to write that as a joke to "I've checked it out, are you happy now?" but then I realized that I would simply be labelled as a spammer rather than a comedian.
Verified game files, updated drivers and general stuff and checked if it happened still?
Yup, everything is in order. And it doesn't happen all the time... in fact it is quite rare... but annoying when it DOES happen.
Well if stuff is updated/verified I've nothing more to add that maybe your OS and computer specs might be thanked if the devs look into this.
Oh and of course, in which maps it happens and at what moment and if something special was happening at the moment and bla,bla,bla

Are you playing co-op? Are you the host? Thinking sync strikes again!
It happens in coop and singleplayer.
Specs:
i7 cpu (forgot which one exactly, will check later when home)
NVidia GTX 460
12 GB ram (1066 if I recall correctly)
Since it's a display issue I'll also point out that I have the nvidia 3d vision 2 and an Alienware Optx aw2310 monitor. Even when not playing in 3d, the desktop runs at 120Hz, and I assume the game does too.
Clearly you need to upgrade your rig.
On a realistic note check v sync and aa settings. It seems to be isolated to your PC unless someone else has it too. Do you have any heat monitoring apps? Maybe the lava is overheating your graphics card. :p
This has been happening to me too recently in single player. I've had to abandon several games because the screen went black after I died. I've messed with my video settings, vsync etc, without any apparent difference.
I also don't have any fancy monitor settings or 3D or anything of the sort.

Not sure what would be causing this. There's nothing obvious jumping out. The easiest way to identify a problem is to reproduce it. That can give us info, but it doesn't sound like there are tried-and-true repro steps here. I'll talk to our testers to see if they've encountered this or have any ideas.
I was playing just now and for the first time i have ever experienced i got this 'black screen of death'. i was playing crossfire endless as the sourcerous (or however its spelt) and got stunned by a frost ogre and then killed by a heavy orc. the death sequence occured but instead of seeing me respawn only the music was heard but i could hear myself shooting and then getting killed/dying again. after that the game was fine again (except the 20+ orcs at the rifts :p ) . i havent updated/changed anything in the game or on my computer and nothing was running in the background.
i also experience the exact same thing. i just bought this game today and only couple hours on. screen turn black when died. controls active( i can see myself running on my coop buddy screen), just screen.
just download the game today, so game data should be recent. evga nvidia 570. intel dual core. 8gb ram. i increased gpu fan speed.
what can i do to provide log?
