Game goes out of sync as host?
I was playing the last level as WarMage with a Shotgun on Nightmare with my friend and at some point towards the end (3rd or 2nd last wave) my grenades stopped firing where my reticule was pointing, instead it kind of rolled them to the lower right of me along the ground and at the same time both my Secondary and Primary attacks stopped hurting enemies. Had to use my Fear and Stun All abilities to give my friend time to come over and finish 'em off for me. It fixed it self when the wave had finished being dealt with.
I also often notice that the shrapnel effect from my my Shotgun when performing Primary Attack often doesn't appear. I just get the spark/sun flair effect with no debris emitting from it.
I am the one that hosts (Invites my friend) so I shouldn't go out of sync I would've thought.
After playing a bit more I assume the games use Steam servers and the online isn't entirely P2P?
Still periodically happens. I've noticed that when it happens the game thinks I am in a different spot to where I actually am, as my attacks occur in a different location to me. Sometimes the game is only slightly out of sync (my shots spawn from beside me), while other times it is very severe (Can be thinking I'm in a totally different place to where I actually am, making it impossible to deal with monsters in any sort of reliable manner).
Unsure what to do about this, it doesn't always happen and tends to fix it self (Or change how badly it is out of sync, at the very least.) at the end of the current wave
I've completed all the Story missions on Normal and Nightmare, all (except the last) Classic missions on Normal and Nightmare, Fire & Water DLC on Normal and that Family DLC on Normal. All of these were completed with me as the host and all of them were done with the same friend. Only I have experienced the game losing sync of where I am so my projectiles spawn from a location I am not at and all the enemies attack where I am not standing yet hurt me.
This happens maybe once every 3? levels.
I did an Endless map with my friend with him as the host and aside from the persistant minor lag the game never considered me as at a location other then where I actually was. However I did notice the enemy AI seemed to have trouble? Like enemies frequently would pause or do seemingly irrational things making it difficult to focus on killing things.
My friend hasn't reported any problems when playing as the client during this entire time we've been playing together so yeah, odd. Other online games work fine like Guild Wars 2, Worms: Armageddon, Duke Nukem 3D, Diablo III, Diablo II (P2P, dunno about Battle.net), Dungeon Defenders, Sanctum, Borderlands 2 etc.
There are a couple of also Maybe-Caused-For-Sync problems around.

I can help little your specific problem by more than connecting it to a maybe bigger underlaying issue with how this game Co-op is built, but still, I would offer you to test with me some endless runs to see if the issue repeats itself or it does not.
I just had the same problem twice in a singleplayer game (weekly challenge, "Not Rocket Science"). Quite troubling if you are trying to kill an ogre. A copy of the shotgun is laying on the ground and any missiles are fired from there. So it might not be a sync-problem
Out of curiosity - were you guys using the Ice Amulet, too?
No, I wasn't using the Ice Amulet. But I found out it happened after being stunned by an ogre.
